         Fall Prevention Program

   Patient safety is our number one goal!  At Trinity Home Health Care, our team approach consists of nurses, physical therapists and occupational therapists working together to develop a personalized plan to improve safety, balance, coordination and ambulation of at-risk patients.

Evaluation and Treatment Includes:

  1. Home safety evaluation 
  2. Balance and strength assessments
  3. Medication review and management 
  4. Patient and caregiver training
  5. Interdisciplinary coordination
  6. Training with assistive devices (wheelchair,  walker, etc)
  7. Training with activities of daily living (dressing, bathing, etc)
  8. Therapeutic exercises addressing balance, strengthening, endurance

  1. Predisposing Factors:

    1. Over 65 years of age
    2. History of fall
    3. Chronic pain
    4. Unsteady gait/balance
    5. Weak muscles
    6. Taking more than 3 medications
    7. Low vision
    8. Dementia
    9. Unsafe home conditions (lack of grab bars, poor lighting, clutter, etc)
    10. Dizziness/vertigo
    11. Poorly fitting footwear
    12. Medical conditions such as:  Low blood pressure, Stroke, Parkinson’s, ALS, arthritis, brain disorders, poorly controlled diabetes, peripheral neuropathy

Quick Facts: 
  1. Risk of fall increases with age and is greater for women than men
  2. ⅔ of people who experience a fall will fall again in 6 months
  3. ⅓ of all falls involve environmental hazards in the home
  4. Falls are the leading cause of death from injury over the age of 65
  5. ¼ of people who fracture a hip die within 6 months of injury- 25% require life-long care
